I don't know about in reality (suppressors are illegal where I live), but, since Metro 2033 is intended to be hyper-realistic (with the exception of the ghosts, mutant animals, and anomalies), I'm guessing that with rifle rounds aside from .300 Whisper, .300 Blackout, 9x39mm, and 12.7x54mm, suppressors make the difference between "There's a guy shooting at us over there!" and "Someone's shooting around here. Don't know where, but I heard gunshots."

The four rounds mentioned and (most) pistol cartridges are much more effective in suppressors. In fact, the quietest guns available are the Welrod (very rare pistol issued to British commandos in WWII in .380 Auto or 9x19 Parabellum), Amphibia-S (integrally-suppressed copy of the Ruger Mark II, which fires .22LR), DeLisle carbine (integrally-suppressed conversion of an SMLE that fires .45 ACP through standard M1911 mags; also very rare), and MP5SD (integrally-suppressed MP5; fires 9x19 Parabellum), which all fire pistol rounds.

Oh, and by the way, the only revolver that can be suppressed and have it work properly is the Nagant 1895, due to the interesting gas seal it has that no other revolver has.

I didn't mention shotguns, because I have no idea how effective shotgun suppressors are. XD

Well shotguns work pretty darn well with CQC combat, however it takes skill to handle a gun like that
